While you may not need the LMN to use your funds initially, it’s wise to keep it readily available, as your HSA/FSA administrator may request it later.

While different plans’ LMN requirements vary, the most essential details are the clinician’s information (name, contact details, license number); your diagnosis (i.e. why you need massage); and the recommended scope of treatment (frequency or number or sessions).
